Position: Community Nurse

Salary: £37,338 -£44,962 per annum

Job Type: Part Time – 30 hours a week

We also provide specialist inpatient services in York and some highly specialised services across the country. Detailed job description and main responsibilities

  • Highlighting and leading upon service improvement initiatives.
  • Providing support for junior staff and learners.
  • Providing evidence-based care.
  • Having experience of time management in terms of a caseload or project work.
  • Timely delivery of the nursing process in terms of assessment, planning, intervention and review.
  • Experience of undertaking various assessment such as biopsychosocial assessments and following clinical pathways.
  • Experience of risk management.
  • Being able to work in a hybrid and flexible way.
  • Look to provide robust managerial and clinical supervision.
  • Professional development through accessing opportunities as identified in appraisals.
  • Developing training tools and providing training as required.
  • Having an understanding of Safeguarding principles.

Qualifications

Essential criteria

  • Qualified and registered RNLD/RNMH

Experience (Essential criteria)

  • Experience of working with people with a learning disability
  • Experience of working with a multi-disciplinary or multi-agency team

Skills (Essential criteria)

  • Must be driver with access to a vehicle.
  • Evidence of professional development

Fixed Term Contracts for existing NHS Employees – temporary contracts for employees of LYPFT, or colleagues joining from another NHS Trust, will be offered on a secondment basis wherever possible. In the event this is not possible, an FTC would be issued and this would require a break in service of two weeks.

The purpose of a temporary is role could be for maternity cover, temporary funding or some other reason. At the end of which the need for the temporary work will cease. Temporary roles can stop, be extended and/or made permanent at any point during the period of work. Should the role become permanent then the post-holder will be informed of the process to be followed to convert the position to a permanent one.
