We have a great opportunity for a full-time band 6 Community Mental Health nurse to work in the Dementia & Older People’s Mental Health Team based in Worthing West Sussex.
The successful candidate will join a friendly, highly-skilled and enthusiastic multi professional team. We will take a keen interest in helping you to develop the skills important to this role and the post-holder will be encouraged to develop special interests and expertise in line with service and team objectives.
You will be part of a highly-skilled and supportive multi-disciplinary team. You will receive regular managerial and clinical supervision. The post holder will be expected to travel across the geographical area ( Adur, Arun & Worthing) covered by the team, the ability to travel to remote areas is essential.

Job Responsibilities
Clinical
- To contribute to the management of a team caseload of clients with complex needs, using CPA principles, evidence based/client centred principles to assess, plan, implement and evaluate interventions, which will be delivered in the Dementia and Older Adults Mental Health Service.
- To support/manage a team caseload and attend relevant MDT meetings
- To regularly support and cover the duty rota
- To provide specialist assessment and intervention as part of a multi-disciplinary approach, with specific responsibilities regarding the care pathway
- To provide risk screening and assessment as well as risk management planning

- To contribute to and facilitate evidence-based assessments and individual and group interventions
- To contribute to the maintenance and development of the Service
- To regularly share and supervise students on practice placement, and junior staff as appropriate
Knowledge/Experience
Essential
- Post qualifying experience in the relevant care group
- Experience of working alongside people with complex health problems in the designated care group
- Experience of providing a range of clinical interventions to people in the designated care group with a variety of health problems
- Experience of working consultatively with professionals
- Experience of conducting clinical assessments including risk assessments
- Experience of partnership working within and across various statutory and non-statutory teams and agencies

- Understanding of clinical governance
- Previous experience of working in the community
