Community Support Worker at Sussex Partnership NHS Foundation Trust

Sussex Partnership NHS Foundation Trust has a great opportunity for a full-time band 3 community support worker to work in the Intensive Support Element of the Dementia & Older People’s Mental Health Team based in Mid Sussex area and will cover Mid Sussex areas in West Sussex. Intensive Support operates an 8am – 7pm shift pattern, with a 1-hr break, Monday to Fridays, 9am – 5pm at weekends and bank holidays. There may be on occasions a need to change this shift pattern to 9-5pm 7 days a week at short notice to meet any changes that may occur in the service.

Main duties of the job

The role of this post is to actively engage those requiring an increased level of support to keep people within their home and maintain their wellbeing and independence as far as possible. We are looking for individuals who are dynamic, compassionate, good communicator who can demonstrate a holistic and person-centred approach to their work.

You will also need to be able to work cohesively with other services both within our Trust, and with other partner agencies. We follow a multi-disciplinary approach; within that context, you will manage a caseload of older adults.

  • Establish relationships with clients and their carers, which are open and two way.
  • Establish methods of communication with clients and their carers, reducing barriers when necessary.
  • Enable clients and their carers to participate in communication concerning their health and well being.

  • Accurately report and/or record work activities according to organisational procedures
  • To support team caseload and attend relevant MDT meetings
  • Assess patients condition through basic test results, assess comfort of patient, and instigate emergency procedures.
  • To contribute to and facilitate evidence-based assessments and individual and group interventions

QUALIFICATIONS

Essential

  • NVQ Level 2 in Care or equivalent experience

EXPERIENCE

Essential

  • Experience working within a health or social care setting
  • Experience of working as part of a team
  • Good communication skills.
